Material Certificates
All of our prosthetics come with a Material Certificate for your records so you and your patients know what materials were used in the manufacture process.
Remake Policy
In the event a prosthesis must be remade, if the doctor takes a new impression, we will charge the doctor 50% of the labor cost. As long as the original alloy is returned there will not be a charge for the new alloy that will be used for the re-make. If the prosthesis fails at time of delivery and there is no new impression necessary, then there will be no charge for the re-make.
Guarantee Policy
Two years on all crowns and bridges, as long as the patients alloy certificate is returned with the failed prosthesis. One year on any laminate veneers. There will be no guarantee or warranty on cement or bonding failures.
Payment Terms
Balance is due the 15th of the month after month of service. A service charge of 2% will be applied to any unpaid balance.
